In 1922, an Iowa native named Charles Osborne was working on a farm in Union, Nebraska, preparing to slaughter a 350-pound hog. As he lifted the heavy animal to hang and weigh it, he collapsed to the ground. Almost immediately afterward, Osborne began to hiccup. While sudden bouts of hiccups are commonplace and typically resolve within a few minutes, Osborne's spasms showed no sign of subsiding as the hours and days passed.
At the onset of the condition, Osborne hiccuped at a rate of approximately forty times per minute. The involuntary contractions of his diaphragm continued day and night, disrupting his sleep, breathing, and ability to eat normally. What initially seemed like a minor post-strain curiosity soon solidified into an unyielding medical mystery that would define the rest of his adult life.
The Physiology Behind the Involuntary Spasm
A standard hiccup occurs when the diaphragm—the primary muscle involved in breathing—undergoes a sudden, involuntary spasm. This rapid contraction pulls air into the lungs, causing the vocal cords to snap shut almost instantly, producing the characteristic 'hic' sound. In most people, hiccups are triggered by minor irritations to the vagus or phrenic nerves, such as swallowing air, consuming carbonated beverages, or sudden temperature changes.
Decades after Osborne's condition began, medical professionals sought a neurological explanation for why his spasms never stopped. A physician who examined him suggested that Osborne may have ruptured a minute, pinhead-sized blood vessel in his brainstem when he fell while lifting the heavy hog.
